Utilizing the Patient Portal is the fastest way to view, download, or print your medical records.
Most of your records are available including:
- Office Visits
- Imaging Reports
- Educational Materials
- Work/School Release Notes
- Messages
- Prescriptions
- Appointment History

Send Your Medical Records to a Healthcare Facility
To request your medical records to be sent to another Healthcare Facility, please log into the Patient Portal and send us a request (the message must include all of the following):
- Dates of service you are requesting
- Specific body part you are requesting
- Name and address of the facility
- Fax number to which to send the records
Note: Orthopedic Institute will fax records unless otherwise requested. Please allow up to seven (7) business days for processing. Failure to provide ALL of the required information may delay processing.

Send Your Medical Records to NON-Healthcare Facility
To request your medical records to be sent to a NON-Healthcare Facility such as your employer, short term disability plan, work comp case manager, lawyer, or other entity, please download, print, and fill out this PDF (Release of Health Information Authorization Form).
Send the completed form to Orthopedic Institute using one of these methods: secure message via the patient portal, fax, mail, drop-off at any OI location, or submit via email.
Send completed form securely via the patient portal:
- To send the form via the patient portal, complete the form and save the PDF to your computer. Then log in to the portal and create a new message. At the bottom of the message you will see the “paper clip” icon. Click on that. Navigate to your saved PDF on your computer and click on it to attach it. Complete your message and press send.

Submit via email:
- Email the completed form to oimedrecords@ortho-i.com
- Submitting via email notice: OI cannot guarantee the security of the email during the transmission process. However, your email usage confirms your consent and understanding that email is a less secure way of transmitting personal information than the other methods provided. Because this email address isn’t bi-directional, you will not receive an email confirmation or response. If we need to contact you, we will contact you via phone or the portal.
