Utilizing the Patient Portal is the fastest way to view, download, or print your medical records.

A staff member will need to access records prior to June in our previous system. Please use a recent version of Chrome, Firefox or Safari.
To request your medical records to be sent to another Healthcare Facility, please log into the Patient Portal and send us a request (the message must include all of the following):
Note: Orthopedic Institute will fax records unless otherwise requested. Please allow up to seven (7) business days for processing. Failure to provide ALL of the required information may delay processing.
